What is Hospice Care?

 Increasingly, people are choosing hospice care at the end of life. Hospice care focuses on the care, comfort, and quality of life of a person with a serious illness who is approaching the end of life.

At some point, it may not be possible to cure a serious illness, or a patient may choose not to undergo certain treatments. Hospice is designed for this situation. The patient beginning hospice care understands that his or her illness is not responding to medical attempts to cure it or to slow the disease's progress.

 Hospice provides comprehensive comfort care as well as support for the family, but, in hospice, attempts to cure the person's illness are stopped. Hospice is provided for a person with a terminal illness whose doctor believes he or she has six months or less to live if the illness runs its natural course.

 It's important for a patient to discuss hospice care options with their doctor. Sometimes, people don't begin hospice care soon enough to take full advantage of the help it offers. Perhaps they wait too long to begin hospice and they are too close to death. Or, some people are not eligible for hospice care soon enough to receive its full benefit. Starting hospice early may be able to provide months of meaningful care and quality time with loved ones. 

What is Palliative Care?

 Palliative care is specialized medical care for people living with a serious illness, such as cancer or heart failure. Patients in palliative care may receive medical care for their symptoms, or palliative care, along with treatment intended to cure their serious illness. Palliative care is meant to enhance a person's current care by focusing on quality of life for them and their family. 

What is 1 Heart?

We at 1 Heart and together with your doctor, a referral can be initiated at no cost to you. A highly trained hospice representative will set an appointment with you and your family at your convenience. This will be a very crucial moment as the most important people in your life, along with the support of a team of health professionals, gather together to uphold what is meaningful to you.

You, as the patient, will be the center of care. Goals will be focused on your preference, dignity and comfort. Plans will be laid out as to how you want your story be told.

Who pays for Hospice of Palliative Care Services?

  • Medicare Hospice Benefit
  • Veteran’s Health Administration
  • Medi-Cal or Medicaid Hospice Benefit
  • Private or Managed Care Insurance
  • Self-pay or Charitable Organizations

 At 1 Heart Hospice and Palliative Care we know you have many options when facing an advanced illness and we are committed to our patients and their families. We offer symptom management and support when you have decided curative treatment is no longer an option. 

A Team with Heart

 1 Heart Hospice and Palliative Care provides treatment of symptoms. This palliative care focuses on enhancing patient’s comfort and overall quality of life and is provided in the patient’s home, a nursing home, or hospital. 1 Heart Hospice and Palliative Care services are available 24 hours a day, seven days a week or on an intermittent basis and include:

  • Attending Physician is the patient's primary care physicians that participates as part of the hospice team.
  • Medical Director certifies the attending physicians prognosis, leads the hospice team in developing a plan of care, provides consultation to physicians.
  • Skilled nurses, who are trained specifically to deal with pain, and symptom management, available 24 hours a day.
  • Social Workers, to offer counseling, and as resources for community services.
  • Hospice Aides & Homemakers
  • Chaplains to provide spiritual support if needed
  • Bereavement Support
  • Volunteers, to provide assistance, through visits, letter writing, reading, or shopping.

Compassionate care is key to a trusted relationship with our team. We not only strive to help you everyday, but want to develop a caring relationship with you. We provide high quality care that cannot compare in other settings. 

Admitting a patient to the hospice program is a simple process for the physician, discharge planner or social worker. If the patient elects the program, the hospice team makes necessary arrangements for admission.

 1 Heart Hospice and Palliative Care recognizes that psychological, spiritual, and social services, as well as medical/clinical care, can contribute to an improve quality of life for patients and their families. A team of dedicated 1 Heart Hospice and Palliative Care professionals, guided by the choices and decisions made by the patient. The team consists of, but not limited to:  

  • The Patient's Physician
  • Skilled Nurse
  • Hospice Aides
  • Social Worker
  • Chaplain
  • Volunteers
  • 1 Heart Hospice Medical Director
